This is a regression from RHEL4.5. Eventually, I tracked down the problem to the device not being configured as a PCI bus master. I have confirmed that this patch fixes the problem, and that reversing the operation reproduces the problem. I have no idea why this patch isn't needed on RHEL4. I will submit this upstream through HP.
